Employee Non-Emergency Accident Procedures

If Medical Treatment is Necessary:

1. Call the SFPD non-emergency line at 352-395-5519. Notify your supervisor.
2. SFPD will complete an Accident/Incident Report and provide you with a letter from our third-party administrator, Davies, that will permit our Workers' Compensation Authorized Medical Provider to deliver medical treatment.
3. Emergency Physicians Medical Center is the college's primary Authorized Medical Provider (see information below). You must provide them with the letter from Davies in order to receive treatment.
4. The provider will determine any future medical needs and/or work restrictions and give you a Form DWC-25. A copy of this form must be given to the Safety and Risk Management Coordinator within 24 hours of this appointment.
5. The Safety and Risk Management Coordinator will file a claim with Davies.
6. If applicable, a Workers' Compensation notification letter will be sent to you and your supervisor with information on work status and/or any necessary follow-up.

If Medical Treatment is NOT Necessary:

1. Call the SFPD non-emergency line at 352-395-5519. Notify your supervisor.
2. SFPD will complete an Accident/Incident Report.
3. If medical treatment becomes necessary, notify your supervisor and the Safety and Risk Management Coordinator.

Student Accident Procedures

If a medical emergency involving a student occurs on campus:

1. Call 911 immediately and the SFPD emergency line at 352-395-5555.
2. Provide 911 with the nature of the emergency, your exact location, and any details that may assist emergency responders.
3. Remain with the injured student until emergency personnel arrive.
4. SFPD will complete an Accident/Incident Report.

If a student injury occurs on campus that does not require emergency services:

1. Call the SFPD non-emergency number 352-395-5519 immediately.
2. If there is any uncertainty whether the incident is an emergency, treat it as an emergency and call 911 and the SFPD emergency line at (352) 395-5555.
3. SFPD will complete an Accident/Incident Report.

Family/Parental Notification

For dual enrollment and charter school students, SFPD will notify parents directly in exigent or emergency situations. In non-emergency situations, SFPD will notify the appropriate charter school or dual enrollment personnel, who will then notify the parents or guardians.

IMPORTANT: College employees may NOT provide personal transportation to injured or ill employees, students, contract workers, or visitors.
