Please choose the appropriate form below to request testing:

Walk-in Testing (Proctoring Request for Course Testing)

Use this form to send your students to the Northwest Campus Testing Center during a specific window of time. This window should be from open to close and preferably more than one day when using the Northwest Campus Testing Center. If you offer the option for students to take course tests on campus or at one of the Centers, complete this form so we can use our bypass code and satisfy your proctoring requirements. Please complete if any of your online students may need to test at one of the centers. Each Center may have its own rules; check with the other Centers you may want to test at before sending a reservation.

Accommodated Walk-in Testing

Use this form to send your Disabilities Resource Center (DRC) students to the Northwest Campus Testing Center during a specific window of time. Please note that this option is only for students that can’t test at the DRC due to conflicts with course schedule or other personal exceptions that have been approved by the faculty.

Whole Class

Use this form to reserve sections of the Northwest Campus Testing Center for your whole class to test. Instructors must accompany the class for an exam and must remain with the students until testing is completed.

Make-up Exam

Use this form to provide information for students needing to take make-up exams at the various satellite Campus Centers or at the Northwest Campus Placement and Assessment Center. Paper-and-pencil make-up exams are no longer available at the Northwest Campus Testing Center. Students should not be directed there for any make-ups.

If you need to extend a testing window for a regular exam in the Northwest Campus Testing Center, reply back to the confirmation email sent by the Northwest Campus Testing Center with this information. Do NOT complete this form.

Please remind students to bring physical ID cards (not cellphone photos of their IDs) when they test with the College. Valid ID s include valid SFID, Driver's License, Military ID or Passports.
