Open enrollment begins October 25, 2021 and ends November 5, 2021.
Summary
Santa Fe College’s annual Open Enrollment period for the 2022 benefit plan year will begin Monday, Oct. 25, and run through Friday, Nov. 5, 2021. Open Enrollment is an opportunity for employees to add new plans, make plan changes, and add or remove dependent and/or domestic partner coverage for the upcoming plan year. No changes can be made after the Open Enrollment period unless they are due to a qualifying event such as marriage, divorce, or birth/adoption. Benefits available during Open Enrollment for the 2022 benefit plan year include medical insurance, dental insurance, vision insurance, flex spending, cancer insurance, sick leave pool, life insurance, disability insurance, and Legal/ID Shield. Rates
Concerning rate changes, Delta Dental rates will have a slight increase for the 2022 plan year on both PPO (Standard and Premier) and HMO plans. Although rates have increased slightly for SF’s group Health Care plans offered through Florida Blue, BlueOptions Traditional PPO 3766, Blue Care-10 Managed Care HMO, and BlueOptions 5190 and 5191 HSA plans, the College will continue to pay 100% of the employee monthly premiums.
Health Savings Account
The BlueOptions 5190 and 5191 HSA plans are a Health Savings Account funded by the College ($500 annually for individual employee coverage or $1500 annually for family coverage). Employees can make additional contributions based on 2022 IRS allowances. If you wish to make contributions, please complete the attached forms and return to Human Resources.
Prescriptions
You can save money on prescriptions by using an in-network pharmacy, which currently includes Walgreens, Costco, Publix, Sam’s, Walmart, and a host of independent pharmacies. You can review the in-network options by consulting this list provided by Florida Blue.
Flexible Spending Account
Custom Benefit Services is now under new ownership, offering additional access and payment options. Please remember that if you wish to participate in the 2022 Flex Spending or Dependent Care plans you must log on to the FBMC portal during the designated Open Enrollment period to make this election. Elections from the 2021 benefit plan year will not carry over into the 2022 benefit plan year. Portal access and registration instructions are available via the FBMC Login.
Cancer Insurance
Enrollment in Cancer Insurance is not currently available through the FBMC portal. To enroll in this benefit, employees must contact Cornerstone Benefits at 1-800-809-8161 during Open Enrollment to add coverage or make changes to existing coverage.
Life Insurance
Effective January 1, 2022, The Standard Life Insurance Company that provides coverage for Supplemental Life/Supplemental AD&D, Dependent Life, Long Term Disability, Short Term Disability, and the College-paid Basic Life Insurance policies will remain the same. Enrollment or changes to current policy would be subject to insurability.
To Enroll or Make Changes
To enroll or make changes to your current health, dental, vision, disability, supplemental life, dependent life, LegalShield, and flex spending plans, you will need to log in to the FBMC benefit portal during the designated Open Enrollment period. Human Resources encourages all employees to log on to the FBMC portal during Open Enrollment, even if no changes are necessary, this is to ensure that your beneficiary information is accurate and up to date.
